New Client Forms - CareNet Counseling

Our counselors and staff want to make it easy for you when it comes to registering as a new client. That’s why we encourage you to take the time to complete the new client forms prior to your initial visit, so you can get started with your counselor right away.

Please Note: If you are coming with someone (i.e. couple or family). ONLY the primary client needs to fill out this paperwork.
